Referrals
For support coordinators, allied health professionals and anyone else referring a participant. Here is what working with us looks like, and what to send in a first message.
What you can expect from us
- Communication. We stay in contact with you and the rest of the participant's team, so decisions are made with the full picture and nothing is lost between people.
- Documentation that is useful to you. Needs, challenges, achievements and support requirements captured across the plan period, in a form that helps with assessments, recommendations and reports.
- A leader who is reachable. Myles reviews documentation, liaises with coordinators and allied health, and still provides direct support himself, so there is one person with the whole picture.
- Goal-focused support that people enjoy. Built around the participant's goals and independence, and delivered through real experiences in the community.
We are a support provider. We do not offer support coordination or plan management, and we work alongside the people who do.
What to include in a first message
- Your name, role and organisation.
- Roughly where the participant lives and how their plan is managed, so Myles can talk about fit straight away.
- The support areas you have in mind, or the goals the participant is working towards.
- The best way and time to reach you.
Please leave diagnoses, NDIS numbers and documents out of a first message. If we are a fit, Myles will arrange a secure way to share what is needed.
