Support to look after your health and wellbeing day to day: getting to the right appointments, keeping up good habits and having encouragement along the way.

About this support

We are not a clinical or therapy service. What we do is help you get to the people who are, and help you keep going with what they recommend between visits.

That might mean getting to the physio and doing the exercises after, a reminder at the right time, a walk or a gym session that becomes part of the week, or simply someone to talk things through with on a hard day.

What this can include

  • Attending medical and allied health appointments
  • Medication prompting and reminders, where appropriate
  • Developing healthy lifestyle habits
  • Exercise and fitness activities
  • Emotional support and encouragement

Medication prompting is only provided where it is appropriate for the person and agreed with the people supporting them.

Part of a connected team

Whatever the support, the way we work is the same. We stay in contact with your family, your support coordinator and your allied health professionals, and we keep documentation that shows what is happening across the plan period. That means the people who write assessments and reports have a clear picture, and you are not the one carrying the story between them.
