Community access and social participation
Support to get out of the house, do the things you enjoy and build real connections with people and places around you.
About this support
Community access is about more than a trip out. It is about having a life in your community: knowing the way to the shops, having a group to go to on a Tuesday, meeting people who share your interests and feeling confident in public spaces.
A support worker comes along for as long as you need them. Over time, the aim is for you to need them less for the things you now do on your own.
What this can include
- Community outings
- Shopping support
- Attending social events
- Recreational activities
- Building friendships and social connections
- Accessing local groups and programs
- Public transport training and support
- Support to attend appointments and meetings
Part of a connected team
Whatever the support, the way we work is the same. We stay in contact with your family, your support coordinator and your allied health professionals, and we keep documentation that shows what is happening across the plan period. That means the people who write assessments and reports have a clear picture, and you are not the one carrying the story between them.
